    The WELL Clinic is a comprehensive and customizable one-year fully telehealth program to help kids and families improve health habits in the following areas: nutrition, physical activity, sleep, screen use, and social and emotional wellbeing. 

    Talk to your child's primary care physician about starting one of the following programs today. Please make sure you have an active MyChart account to receive more information.

    The following options are available for patients referred to The WELL Clinic.   

    WELL Clinic:

    The WELL program is more intensive and includes a monthly video visit for 12 months with a focus on tailored goals around nutrition, physical activity, sleep, screen use, and social-emotional wellness. Visits are with a pediatrician or nurse practitioner who specializes in nutrition and weight management. WELL includes optional Virtual Teaching Kitchen and/or health coaching add-on for additional support. Additionally, patients considering medication or surgical options for obesity management may be referred to WELL.

    Nutrition Consult:

    Nutrition consults are available for specific nutrition-related concerns with referral from a CPCMG pediatrician. Nutrition consults are with Dr. Natalie Muth who is a board-certified pediatrician and obesity medicine specialist and a registered dietitian nutritionist.

    Healthy Living Visit (HLV):

    A six-session, six-month program with your own pediatrician or a clinician based out of your home site or region. These appointments are available in person or through video visits. HLV includes theme-based visits and goal setting on key nutrition, physical activity, sleep, screen use, and social-emotional wellness topics.

    Virtual Teaching Kitchen (VTK):

    Nutrition and cooking telehealth visits taught by pediatrician and culinary medicine expert, Dr. Mary Tanaka. Class topics include making vegetables delicious, cooking with whole grains, healthier sweets, and more. Also available are tips for cooking on a budget and picky eating.

    CALM:

    CALM stands for Connection Awareness Lifestyle Mindfulness. It is rooted in positive psychology and takes a strength-based approach to helping patients manage depression and anxiety. It is a coaching program and is not intended to replace therapy.

    The program includes 4 sessions and are individual 20-minute telemedicine appointments with NP Nicole Kastner who has extensive training and experience in mindfulness including mindfulness educator certificate for children K-12 and 25 years of licensed massage therapy and kids yoga.

    The program includes:  Mindfulness /Wheel of Health; S.T.O.P. and 3 Good Things; Savoring; and Mindfulness Practice. CALM is ideal for patients ages 5 and older.

    MOVE:

    MOVE stands for Moderate-intensity, Outdoors, Variety, and Enjoyable -- key targets for a fun and effective physical activity program. MOVE includes a telehealth consult with Dr. Natalie Muth and either one on one telehealth visits or group telehealth visits with Senior WELL Clinic Educator and Personal Trainer Katie Diaz.

    Scheduling

    Once you are referred to a WELL Clinic program by your Primary Care Physician, you will receive a MyChart message with scheduling details. You may self-schedule through MyChart or call 760-633-3640.
