Your Upcoming Visit
How much face-to-face time is scheduled with the health care provider? Most Initial visits include 30-60 minutes with the pediatrician or advanced practice provider (APP). Most Follow-Up visits include 20-30 minutes with the pediatrician or APP. Some brief Medication Check visits are scheduled for 10 minutes.
Who should attend? We invite all parents or primary caregivers who live with the patient to join the visit. This helps the health care team collect valuable information and develop a treatment plan tailored to your child and family. At a minimum, the patient and one parent or caregiver must be present. Whenever possible, please do not bring siblings to these visits.
Will I be able to speak with the health care team without my child present? During these visits, the health care team will provide an opportunity for parents/caregivers to discuss concerns without the patient present. The team will also allow patients to discuss concerns without their parent/caregiver present, when age-appropriate.
What should I bring? A list of your questions will help guide the visit. A copy of the latest note or evaluation report will be essential if your child has been evaluated by an outside psychiatrist, psychologist, or counselor. Also, please bring a list of all current medications that your child is taking. Please provide a copy if your child already has an individualized educational plan (IEP) or a 504 plan. Please bring larger documents to your in-person appointment or send brief documents via MyChart.
Do I need a MyChart account? Yes. MyChart allows you to complete pre-visit questionnaires and communicate with your health care team between visits. Also, we ask all patients 12 years and older to activate a Teen MyChart account. Please call your CPCMG office to sign up if you don't have an account.
How do I complete pre-visit questionnaires? Questionnaires are available on MyChart on a computer web browser and the MyChart mobile phone application seven days before your visit. You will find questionnaires in the Menu section.
How can I make the check-in process easier? EZ Arrival is available on MyChart two days before your visit. Please access this activity on the homepage to update your information, submit any required copay, and complete the remaining questionnaires. If you are unable to complete the questionnaires beforehand, please arrive at least 30 minutes before the appointment start time.
If my visit is via telehealth, how do I connect? We request you log on to MyChart from your phone 10-15 minutes before your appointment. When you select the patient and scroll down to the visit, you'll see an EZ Arrival or Start Visit button.
Will I have access to notes written by the pediatrician or APP after the visit? Due to federal and state privacy laws, visit notes are only available to parents and legal guardians for children less than 12 years old.
How often will we have follow-up visits? Your upcoming visit is part of a care journey that will include follow-ups to collect additional information and adjust any treatment plan. Initially, some follow-ups may occur every 1-2 weeks. Then, a follow-up visit every 3-6 months is required if medications are prescribed. Many of these follow-up visits will be available via telehealth. We request that you always schedule the follow-up visits with the clinician who regularly manages your child’s medication.
Will the health care team want to talk with teachers or therapists? Your clinician may request to speak to your child’s teacher or therapist after the visit if they feel it will help develop a treatment plan for your child.
