    How to upload documents to a patient's health record

    This article provides step-by-step instructions on uploading clinical photographs and documents to a patient's health record

    You can upload documents to a patient's health record. Examples include school forms needing completion and summaries of care from another health care provider. This can be performed from a computer web browser or the MyChart app on your phone.

    Upload a document from a computer web browser or MyChart phone app.

    First, log on to MyChart on a modern web browser or the MyChart app on your phone.

    1. Select Messages.

    2. Then, Send a message.


    3. Select "Medical Question."


    4. Select "Non-Medical Question". Please note that documents uploaded in this fashion will NOT be reviewed by a doctor or APP unless a visit occurs, and are only for purposes of sending school forms to be completed or to be included in the patient's chart. For clinical questions or concerns, please select Medical Question (Non-Urgent).


    5. Select the clinician's office where you want the documents reviewed. Documents will be available from all CPCMG locations, but this choice notifies a specific office that an upload has occurred.


    6. Enter a brief Subject and Message, click the paperclip icon to select specific files to upload, then click Send.
