Join Us in Shaping the Patient Experience at Children's Primary Care Medical Group (CPCMG)!

What is the Patient & Family Advisory Council (PFAC)?
The PFAC is a partnership between families, caregivers, and healthcare team members at CPCMG. Our goal is to gather insights directly from the people we serve to improve the care experience for all patients and families. Your voice matters, and we want to hear from you!

     Why Join the PFAC?
    - Make an Impact: Share your ideas and experiences to help shape policies, services, and care delivery.
    - Collaborate: Work with healthcare leaders and other families to develop new solutions and improvements.
    - Be Heard: Your insights will directly influence how we deliver care and design patient-centered services.

    What Does Participation Look Like?
We offer two ways to get involved:

    1. Virtual Meetings: Join us via Zoom for discussions, updates, and brainstorming sessions. Meetings are held four times per year and last about 60 minutes.

    2. Online Surveys: Participate on your own time through surveys. We’ll provide questions on key topics and welcome ongoing feedback.

What is Expected of PFAC Members?
    - Attend virtual meetings and/or respond to surveys. We conduct Zoom meetings four times per year and release new surveys every few months.
    - Provide honest feedback based on your experiences with our services.
    - Respect others’ opinions and work collaboratively toward improving care for all families.

What Are the Benefits of Joining?
    - Influence the future of pediatric care in your community.
    - Build connections with other families and our healthcare team.

    Who Can Join?
We are looking for parents, caregivers, and teen patients who:
