# How to Request Medical Records in MyChart

1. On Main Menu, Go to My Record>Request Medical Records

1. Select the appropriate organization you would like to request records from.
2. Answer all questions regarding who records should be sent to, date ranges, and checkbox all applicable records requested:

1. Restrictions regarding release of sensitive medical information will appear, read through and select options if applicable:

1. Manually write method of delivery (ex. Printed and picked up)

1. Continue and acknowledge review screen to submit your request.
